Prize Drawing Results for the 2025-26 Academic Year Administrative Satisfaction and Service Quality Survey（English Version）

感謝全校教職員生踴躍參與「114學年度行政滿意度暨服務品質問卷調查」。本次問卷調查抽獎結果已公告，請獲獎者依下列說明辦理領獎事宜。

一、領取時間：
115年6月5日（星期五）至115年6月26日（星期五）止。
（限平日上午09:00至12:00、下午2:00至5:00）

二、領取地點：
國研大樓4樓4003室（永續發展辦公室 校務永續組）

三、領取方式：
請獲獎者攜帶學生證、職員證或其他相關身分證明文件辦理領獎（如需委託他人代領，請以領獎通知電子郵件回覆委託書）。
另，iPad 11及Garmin智慧錶款之獲獎者，請務必攜帶身分證明文件辦理領獎（此二項大獎建議由本人親自領取）。

四、注意事項：
請於115年6月26日（星期五）前完成領獎。逾期未領者，視同放棄領獎權益。如於公告領獎期間不克前來，請事先與本辦公室聯繫，並告知預計領獎日期，感謝您的配合。

Announcement of Prize Drawing Results for the 2025-26 Academic Year Administrative Satisfaction and Service Quality Survey

Thank you to all faculty, staff, and students for participating in the “2025-26 Academic Year Administrative Satisfaction and Service Quality Survey.” The prize drawing results have been announced. Winners are requested to claim their prizes in accordance with the following instructions.

1. Prize Claim Period
From Friday, June 5, 2026, to Friday, June 26, 2026.
(Prizes may be claimed on weekdays from 09:00 to 12:00 and from 14:00 to 17:00.)

2. Prize Claim Location
Room IR4003, 4th Floor, Building of International Research
(Division of Sustainable Institutional Research, Office of Sustainability)

3. Prize Claim Method
Winners are required to present their student ID card, staff ID card, or other valid identification documents when claiming their prizes.
If a winner wishes to authorize another person to claim the prize on their behalf, please reply to the prize notification email with an authorization letter.
Winners of the iPad 11 and Garmin smartwatch prizes must bring valid identification documents. It is recommended that these two grand prizes be claimed in person by the winners themselves.

4. Important Notes
Please complete the prize claim process by Friday, June 26, 2026. Winners who fail to claim their prizes by the deadline will be deemed to have forfeited their right to claim the prize.
If you are unable to claim your prize during the announced prize claim period, please contact us in advance and inform us of your expected claim date by email at duan@mail.nsysu.edu.tw. Thank you for your cooperation.
